Apple Health Six-Month Renewals and Work Requirements: A Guide for Washington Clinics
Apple Health for Adults moves to six-month renewals and work requirements on January 1, 2027, with self-attestation accepted through 2027. Here's what HCA has decided and what's still open.
Updated November 4, 2026, based on the Washington State Health Care Authority (HCA) as of that date.
Apple Health for Adults moves to six-month renewals on January 1, 2027. Coverage periods drop from 12 months to six at each person's first renewal in 2027, and most people applying from that date get a six-month certification period. Work requirements also start January 1, 2027, and the Health Care Authority (HCA) says the first renewal month they apply to is February 2027.
Through 2027, HCA will accept self-attestation of work activities and exemptions, checked against data where it can, with stricter verification in 2028. The changes come from H.R. 1 and apply to adults 19 to 64 without Medicare and with income under 138% of the federal poverty level. For the federal rules behind them, see our guide to the federal changes.
When each change starts in Washington
- Six-month renewals: January 1, 2027, at each person's first renewal that year, per HCA's work requirements and renewals fact sheet (June 2026).
- Work requirements: January 1, 2027 for applicants. The first renewal month they apply to is February 2027, per HCA's September 2026 H.R. 1 webinar slides.
- Shorter retroactive coverage: January 1, 2027, per HCA's retroactive coverage fact sheet.
- Verification beyond self-attestation: 2028, per HCA's September 2026 webinar FAQ.
American Indian and Alaska Native individuals are excluded from both work requirements and six-month certification periods. Work requirements don't apply to any other Apple Health program, including children, pregnancy services and long-term care.
How Washington will renew Apple Health for Adults
The renewal process itself isn't changing. In the fact sheet's words, "The system will continue to attempt an automatic renewal before sending a notice for the individual to take action to keep their coverage." HCA's example: someone approved through Washington Healthplanfinder on February 1, 2027 has a certification period ending July 31, 2027, not January 31, 2028.
The September slides put the affected group at roughly 610,650 people, based on June 2026 caseload. Almost 80% are renewed automatically today, and about 20% must renew manually. HCA expects the shorter periods to add burden for that 20%.
Washington will send the work requirement noncompliance notice with the prepopulated renewal form, not after the form comes back.
How Washington will check work requirements
Under HCA's work requirements page, people qualify with at least $580 in monthly household income (or a $580 six-month average for seasonal workers), half-time school, 80 hours a month of a work program or community service, or 80 hours combining work, service, a work program and school.
The look-back period depends on the event:
- At application: the month before the application month.
- At renewal: any one month during the certification period.
Among Washington's other choices in the slides, it won't verify compliance more often than required. It will use all four optional short-term hardships, and it defines "outside of your community" as a commute of over an hour. The FAQ adds that anyone living in a county identified as having high unemployment qualifies for that exemption automatically.
For verification, HCA says it "will continue to accept self-attestation of your circumstances and will utilize data sources to verify that attestation in 2027 and at your first renewal in 2028." In 2028, people may need to send more proof when data can't confirm what they reported. Medically frail patients will answer a screener in Washington Healthplanfinder. Starting January 2028, HCA will check that status against its code list using claims, which takes "a diagnosis and one inpatient or outpatient claim in the 12-month lookback period."
Exemptions in the fact sheet include pregnancy and the 12-month postpartum period, former foster youth under 26, parents and caretakers of a child under 14 or a person with a disability, SNAP or TANF household members, Medicare, recent release from incarceration, medically frail status, AUD or SUD treatment, and 100% disabled veterans.
What changes for providers and health centers
Retroactive coverage shrinks on January 1, 2027. Apple Health for Adults gets one month before the application filing date, and all other Apple Health programs get two, down from up to three. A patient who applies in January 2027 can get December 2026 covered under Apple Health for Adults, or November and December under other programs.
To see whether a patient is on Apple Health for Adults, the slides point to the ProviderOne benefit inquiry screen: coverage group code N05 with Recipient Aid Category (RAC) 1201 or 1217. That is the group the six-month renewals and work requirements apply to.
HCA planned to notify more than 520,000 households in September 2026, starting the week of September 8, with texts as a second channel. Initial letters continue through January 2027. The FAQ says the letter "is informational only" and goes to every Apple Health for Adults recipient, including people who are exempt.
HCA's H.R. 1 impacts page links a provider toolkit, slides from provider webinars on September 15 and 29, 2026, and a Community Connector program with monthly updates. A fourth quarterly webinar is scheduled for December 3 at 10 a.m.
What Washington hasn't decided yet
The FAQ opens with "All the information presented in this FAQ is preliminary and subject to change." HCA lists these as still open:
- Which Washington programs count toward work program hours, and which count as AUD or SUD treatment. The slides mark both "Coming soon."
- How it will verify activities, exclusions and exemptions in 2028. For caregivers, the FAQ says "We do not have the verification process in place for this currently."
- The Washington Healthplanfinder application questions. The FAQ says "The changes to the Washington Healthplanfinder application are still underway."
HCA's documents point in different directions twice. The impacts page lists work requirements as "January 1, 2027, with option to apply for waiver to implement December 31, 2028." The September slides say "Washington elected to apply work requirements on or after January 1, 2027," and HCA's documents don't mention a request to delay. Plan for January 2027.
The same slide deck says both that "HCA published a code set to verify Apple Health for Adults medically frail attestation" and that the conditions and billing codes that verify the exemption are "Coming soon." Ask HCA which list is final before you build claims checks around it.
Frequently asked questions
Does a patient approved in February 2027 renew in six months?
Yes. HCA's example has a February 1, 2027 approval ending its certification period on July 31, 2027.
Will patients have to prove their work hours in 2027?
HCA will accept self-attestation in 2027 and check it against data sources. Additional proof may be needed starting in 2028 when data can't confirm the attestation.
How far back does the work requirement look at renewal?
Any one month during the certification period. The FAQ says "The requirement to meet in the month before is only at the time of application."
